Includes everything in the Pilot Basic Program, plus:

• Weekly Check-Ins — More frequent monitoring of health, mood, memory, and safety, so changes are caught early.
• Caregiver Coaching — One-on-one guidance and emotional support for family caregivers, helping reduce stress and burnout.
• Hospital Discharge Support — We can help the family coordinate the hospital discharge details, ensuring a smoother, safer transition from hospital to home.

Payment Terms:
This is a one-time monthly payment plan — no long-term contract required. Families can choose to continue month to month or stop at any time, giving you full flexibility to adjust your support level as circumstances change.
